Facilitation

Montana Perinatal Quality Collaborative (2020 - Present)

Yarrow is facilitating the statewide Perinatal Quality Collaborative as it implements the Alliance for Innovation on Maternal Health (AIM) bundles with the goal of improving maternal health outcomes. This quality improvement project utilizes expert and peer-to-peer learning models from the Institute for Healthcare Improvement (IHI) Breakthrough Series Model for Quality Improvement. 


Montana WIC Workgroup (2020 - Present)

Bringing together representatives from the Montana WIC program to identify and complete program improvement projects. 


Montana Children’s Special Health Services - Montana Access to Pediatric Psychiatry Network (MAPP-Net) (2020 - 2021)

Providing meeting planning and facilitation services for the MAPP-Net Advisory Council’s annual meeting to assess progress and determine future projects.


Montana’s Newborn Screening Advisory Committee (2022 - Present)

This Committee has been convened to ensure that Montana has a clear and transparent process for determining which conditions should be added to the statewide newborn metabolic screening panel.


Evaluation

Good Health & Wellness in Indian Country, Fort Peck Community College (2019 - Present)

This CDC-funded 5-year project aims to increase physical activity and improve clinical-community linkages for people at risk for diabetes and cardiovascular diseases. Yarrow provides evaluation support with evaluation plans, data visualization, and continual analysis to drive improvement in activity implementation and outcome success. 

Montana Oral Health Program (2020 - Present)

Yarrow has been brought on to develop, implement, and monitor a comprehensive evaluation plan for the Montana Innovations in Prevention Grants to States to Support Oral Health Workforce Activities (HRSA 18-014). 

Montana WIC Integrated Nutrition Access Program (2021 - Present)

Yarrow is working with multiple partners across the state to create evaluation and quality improvement plans for a collective impact-based initiative to integrate WIC services into pediatric clinics. 

FAST Blackfeet Nutrition Education Programming (2021 - Present)

Yarrow assists FAST Blackfeet by capturing different data points from multiple grants and tracking them as a one large evaluation plan that can be used for reporting to grantors and stakeholders.

Montana WIC Program Statewide Needs Assessment (2020 - 2021)

Yarrow conducted the first statewide needs assessment for the Montana WIC Program, utilizing data from a variety of sources and the federal Nutrition Services Standards to develop and structure the project.

Idaho Statewide Opioid Misuse and Overdose Strategic Plan (2020 - 2021)

In 2020 Yarrow was brought onto the ODP Strategic Planning team to continue the annual updates of Idaho’s Opioid Misuse and Overdose Strategic Plan (2017 - 2022). This process included in-depth team-based planning and preparation for the virtual facilitation of the annual one-and-a-half-day meeting, which was attended by more than 40 organizations each year.


COVID-19

Montana Vaccine Equity Toolkits (2021)

Through work with the Montana Public Health Institute, Yarrow created a Vaccine Equity Toolkit to assist small and medium-sized counties with improving COVID-19 vaccination rates. Along with creating the Toolkit, Yarrow offered 1:1 Technical Assistance to these counties in improving implementation of vaccine related interventions using quality improvement tactics. 

Blackfeet Nation COVID-19 Technical Assistance (2020 - 2021)

The Blackfeet Nation contracted Yarrow to continue the work provided under a previous technical assistance contract with the State of Montana. Yarrow developed a COVID-19 strategic response plan, Phase Plan, grant application and receipt tracking system, program financial need system, program work plan templates, and ongoing public health data collection, synthesis, communications, and planning guidance for the Blackfeet Nation.

Small Counties in Montana (2020)

Early in the pandemic, the Montana DPHHS contracted Yarrow to provide immediate technical assistance to Montana communities experiencing outbreaks of COVID-19. Yarrow was able to streamline public information sharing, including timely press releases and social media posts, establish data tracking protocols, apply for additional funding support, and assist in contact tracing. Additionally, Yarrow worked across several jurisdictions simultaneously to streamline joint information sharing, establish data-tracking protocols, advise and create systems for working with community schools, tracking hospitalizations and deaths, writing policies for local departments, and more.


Tribal Health Technical Assistance

Tribal ISDEAA/638 Contracting (2021 - Present)

Yarrow continues to work with the Blackfeet Nation's Tribal Health System to contract health services from Indian Health Services to be managed by local tribal leadership. This experience includes proposal development, submission, and negotiation. 

FAST Blackfeet & Tribal Health Services Partnership (2020 - 2021)

This project weaves together an innovative partnership between the non-profit, FAST Blackfeet, and the Blackfeet Tribe's healthcare system to provide patient access to Medical Nutrition Therapy and integrated culturally relevant nutrition education.


Project Management

Blackfeet Integrated Perinatal Mental Health, Meadowlark Initiative (2019 - Present)

The Meadowlark Initiative is a statewide initiative of the Montana Healthcare Foundation and the State Medicaid Office to support intensive care coordination and integrated mental healthcare for pregnant and postpartum women. Yarrow provides project management support to the implementation team at Blackfeet Community Hospital and Southern Piegan Health Center. 

Blackfeet Journey to Recovery Center (2019 - 2020)

Yarrow worked with the JTRC, a medication-assisted treatment clinic, to develop workflows, billing, and revenue streams; write policy; and establish a robust evaluation and quality improvement program. 

Northern Cheyenne School Based Health Initiative (2021 - Present)

The Northern Cheyenne School Based Health Initiative is an impressive expansion of Northern Cheyenne Tribal Health, in partnership with Montana State University’s School of Nursing and Smiles Across Montana, to provide primary care, dental services, and mental healthcare in four school locations across the Northern Cheyenne Nation. Yarrow provides project management support to the implementation team.



Research

Montana Opioid Response Research and Evaluation Study (2020)

The Addictive and Mental Disorders Division of the MT DPHHS contracted JG Research and Evaluation out of Bozeman, MT to conduct a multi-part study on the State’s Opioid Response grant (SOR). Yarrow was subcontracted to conduct portions of the stigma section of the larger study. This consisted of conducting a multitude of interviews in two rural locations within the State of Montana, which were then coded, analyzed, and reported to the State. Plans are in progress to develop the study results into publications.


Grant Writing

Yarrow has written multiple grants resulting in over $2 million of funding for various Tribal Nations and Counties. Grant topics have included youth homelessness, substance use disorder treatment, mental health, school-based health, and COVID-19 response support.
